编者按:目前,辅助治疗已成为高危复发的早期结直肠癌患者的标准护理方法。循环肿瘤DNA(ctDNA)已被证实可检测术后微小残留病(MRD),并从低风险患者中识别出高危患者,从而预测复发。本文中,马拉维Kamuzu中心医院-国家癌症中心的Precious Takondwa Makondi教授阐述了结直肠癌的辅助治疗以及MRD的作用。

利用临床病理特征选择将从辅助化疗获益的早期结直肠癌患者与发现疾病复发率高相关。
循环肿瘤DNA(ctDNA)已被证实可以检测术后微小残留病变(MRD),从低风险患者中分层高危患者,并预测复发。
结直肠癌(CRC)是全球第三大常见癌症,也是癌症相关死亡的第二大常见原因[1]。早期结直肠癌(Ⅰ~Ⅲ期)唯一的治愈方式是手术[2]。然而,约20%~30%的患者会发展为肿瘤复发和转移性疾病并最终死亡,其中的一个原因可能是由于在常规分期中微小转移灶未被识别出来[3]。因此,为防止疾病复发,辅助治疗成为高危复发的早期结直肠癌患者的标准护理方法[4]。
按照疾病分期的护理治疗标准
对于Ⅲ期结直肠癌,推荐进行辅助化疗,这可减少约30%的复发率[5]。对于Ⅰ期结直肠癌,由于单纯手术的5年生存率就可达到99%,因此并不推荐进行辅助治疗[2]。对于Ⅱ期结直肠癌是否进行辅助化疗尚存争议。风险评估是确定Ⅱ期结直肠癌是否需要辅助治疗的一个重要方面[6]。无高危特征(包括高频微卫星不稳定[MSI-H]或错配修复缺陷dMMR)的患者通常不需要辅助治疗[7]。具有下表中所列临床特征的MMR表达正常(pMMR)的患者被认为是高危患者,应该接受辅助治疗[8]。

在Ⅱ期结直肠癌中,5-氟尿嘧啶(5-FU)和叶酸常作为标准用药,卡培他滨可作为替代用药[4]。在以氟嘧啶为基础的辅助治疗中常规添加奥沙利铂已被证明没有生存效益,但如果患者有多个高危因素,则可以添加奥沙利铂[9]。dMMR/MSI-H疾病患者不会从5-FU辅助化疗中获益,但如果这些患者有高危因素,一般决定进行辅助治疗,并推荐使用奥沙利铂[4]。
在Ⅲ期结直肠癌中,根据复发风险,推荐的标准辅助治疗方案是3或6个月的5-FU/亚叶酸(LV)/奥沙利铂(LV钙、FU和奥沙利铂)或卡培他滨/奥沙利铂(卡培他滨和奥沙利铂/卡培他滨和奥沙利铂)[10]。
在早期结直肠癌中,为了最大限度地提高生存率和发现早期复发,建议在术后进行监测(包括临床评估、X线、内窥镜和/或癌胚抗原评估[CEA])[11]。尽管如此,即使采用最佳方案,仍有一半的结直肠癌病例将发展为无法治愈的转移性疾病[12]。目前评估复发风险的方法预测价值较差,导致许多患者治疗不足或过度治疗[13]。这导致辅助治疗的目标,即消除术后微小残留病(MRD),仍然无法实现。对于没有高危特征且未接受辅助治疗并随后复发的患者也是如此,因为如果发现了MRD的证据,这些患者可能会从辅助化疗中受益。
支持使用ctDNA驱动的治疗决策的数据
ctDNA已被证明可以可靠地提示治愈性手术后MRD的存在,在预测复发方面具有良好的敏感性和高特异性[14]。两种最常用的ctDNA检测是Reveal和Signatera。Reveal是一种肿瘤不知情分析(tumor agnostic assay),可以同时评估基因组突变和甲基化,以检测残留病灶和疾病复发,而Signatera是通过对个体原发肿瘤的全外显子组测序获得信息。ctDNA的检测可以预测预后,这在目前的方法是无法做到的[15]。
在Reinert等人的一项研究中,94例Ⅰ~Ⅲ期结直肠癌患者中,84例(89.4%)ctDNA阴性,其中只有10例(11.9%)发生复发。在10例ctDNA阳性的患者中,有7例(70%)复发。在同一项研究中,当给予辅助化疗时,只有那些ctDNA未清除的患者(4例)出现了疾病复发,证明在Ⅰ期至Ⅲ期结直肠癌患者的随访中ctDNA具有强大的预后作用[16]。Tie等证实,无高危特征且未接受化疗但在治愈性手术后检测到ctDNA的Ⅱ期结直肠癌患者,其疾病复发的可能性是未检测到ctDNA的患者的18倍(79% vs. 9.8%)[16]。DYNAMIC试验还表明,ctDNA引导的化疗选择策略可能在Ⅱ期结肠癌患者人群中获得与目前使用的临床特征相似的结果,且化疗暴露率较低[17]。然而,本研究未能提供化疗治疗效果的评估,以帮助指导辅助化疗决策的制定[17]。GALAXY研究证明了ctDNA动态与改善MRD阳性患者临床结果的相关性。该研究能够确定所有分期的哪些患者可能从辅助治疗获益[18]。
Henriksen等人最近在Ⅲ期结直肠癌中证实,ctDNA是手术和辅助化疗后复发的一个强有力的预测因子。在ctDNA阳性的患者中,80%复发,而ctDNA阴性的患者中只有18%复发。研究还表明,当ctDNA阳性的患者接受化疗时,所有永久清除ctDNA的患者都不会复发,而所有未清除或暂时清除的患者都出现复发。辅助治疗后ctDNA持续的患者至复发的时间更短。当与目前的随访方法相比时,与CEA相比,ctDNA阳性与无复发生存期显著相关,从术后系列的ctDNA检测到MRD,到发生可通过标准放射成像技术被检测到的复发,中位时间为8个月(0.56~21.6个月)[19]。
然而,与这些发现相反,Fakih等人证明,ctDNA分析在检测疾病复发方面可能并不优于目前的影像学和CEA方法,因此其在临床决策中并不可靠。在48例可切除的Ⅱ至Ⅳ期结直肠癌患者中,15例复发的患者中,ctDNA仅能在5例患者中检测到MRD,而ctDNA阴性的15例中,影像学和CEA检测到7例复发[20]。这些研究的主要区别是放射线监测的时间间隔。Fakih等采用国家综合癌症网络的CEA和CT扫描指南,每3~6个月扫描一次,持续2年,之后每6~12个月扫描一次,持续3年。然而,在欧洲的研究中,西班牙的护理标准是每6个月检测一次,丹麦是在12个月和36个月时各检测一次。这可能是在欧洲研究中ctDNA优于影像学监测复发,而在美国研究中则不是的原因。
总结
使用ctDNA检测结直肠癌中的MRD已明确显示出在指导辅助治疗决策方面的益处。此外,ctDNA可预测治疗反应和疾病进展,因此可以避免不必要的化疗暴露,同时避免治疗不足。ctDNA还有助于患者随访和早期发现疾病复发。许多具有不同终点的研究仍在进行中,包括CIRCULATE[21]、BESPOKE[22]、TRACC[23]、VEGA[24]和IMPROVE-IT2[25]。这些随机试验评估了ctDNA在辅助化疗决策中的作用,BESPOKE试验和IMPROVE-IT2试验则也对放射学监测进行了评估。因此,等待将ctDNA检测纳入目前的早期结直肠癌辅助治疗指南是有益的。
