结直肠腹膜转移:基因组学揭示器官特异性微环境是开发新疗法的关键

编者按:腹膜转移是结直肠癌(CRC)中常见的转移性扩散类型,且预后较差,随着新型免疫治疗试验的开发,更好地了解免疫微环境在腹膜转移中的基因组差异和相应影响可能会显著改善患者的预后。德克萨斯大学MD安德森癌症中心结直肠外科Abhineet Uppal博士和George J. Chang博士在本文就结直肠腹膜转移的基因组学进展进行了综述。
 
腹膜转移是结直肠癌(CRC)中常见的转移性扩散类型,高达20%的局部进展患者在根治性手术后发展为腹膜转移。尽管临床试验表明细胞减灭术可改善中位生存期,并且完全减灭后的长期生存期是可能的,但临床复发很常见[1,2]。此外,只有一小部分腹膜转移患者适合手术,因此,全身治疗仍然是治疗腹膜转移的重要组成部分[3]。

为什么腹膜转移患者的总生存期更差?
 
单纯肝或肺转移患者与腹膜转移患者对全身治疗的反应和总生存期差异显著。消化系统癌症分析与研究(ARCAD)合作组织 2016年的一项综述显示,腹膜转移患者的中位总生存期仅为16.3个月,而仅肝转移患者的中位总生存期为19.1个月,纯肺转移患者的中位总生存期为 24.6个月(HR分别为0.75和0.53;P<0.0001)[4]。这种生存率下降的原因可能是肿瘤对细胞毒性治疗的反应降低和治疗限制性并发症(如恶性肠梗阻)的发生相关。右侧肿瘤、T4期、淋巴结阳性和组织学特征(如印戒细胞)都与腹膜转移有关,导致这些原发性肿瘤特征的总生存期较低[5,6]。
 
肿瘤侧向可能结合BRAF和KRAS状态影响靶向治疗的使用,并且对T4原发性肿瘤使用预防性腹膜内热化疗进行了研究,但结果相互矛盾。虽然我们可以预测哪些患者有腹膜转移的风险,但需要更有效的方案来控制腹膜转移,并可能诱导足够的反应来帮助患者成为细胞减灭的候选者。
 
了解器官特异性微环境是开发新疗法的关键
 
自ARCAD审查以来,CRC的全身治疗不断发展,微卫星稳定CRC的靶向治疗和MSI-H CRC的免疫治疗都有所改善,后者的反应显著。在特定的KRAS突变亚型中,新型KRAS抑制剂显示出希望,包括在1/2期试验中KRASG12C突变CRC的疾病控制率为74%~87%7,8]。超过一半的腹膜转移患者会出现KRAS突变,为KRAS抑制剂或其他RAS通路效应子的开发提供了丰富的靶点环境[9]。
 
同样,BRAF V600E突变患者从BRAF抑制剂中获益,如BEACON CRC 联合BRAF和EGFR抑制试验所示[10]。这一发现对于腹膜转移尤其重要,因为BRAF V600E突变与这些患者的生存率显著降低有关(HR 4.2; P=0.002)[11]。随着全身治疗选择的增加和与位点无关的突变特异性试验的发展,了解驱动突变、基因组性状和腹膜转移的其他特征的差异对于临床照护和试验开发将变得越来越重要。
 
最近报道的新型检查点阻断方案(如瑞戈非尼加伊匹木单抗和纳武利尤单抗,以及博替西单抗加巴司利单抗)的I期试验的早期结果最能说明了解转移瘤的器官特异性微环境的重要性[12,13]。两项试验在没有肝转移的患者中的疾病控制率分别高达82%和93%;这些比率明显高于肝转移患者,非常期待这方面的其他数据。这些临床上相关的差异与最近的发现一致表明肝转移会消耗体循环中活化的CD8 + T细胞[14]。

此外,最近发表的腹膜转移分析表明,CMS4亚型的优势与抑制免疫反应的调节性T细胞和巨噬细胞的浸润有关[9,15]。最近报道的免疫治疗方案可能克服这些抑制机制,导致观察到对腹膜和其他肝外疾病的活性。在免疫治疗试验中对治疗转移瘤的进一步研究可能会确定疾病控制的其他协同机制。随着可用于转移性结直肠癌的药物数量的增加,结合转化研究以了解特定部位反应的临床试验将有助于优化患者护理。
 
这些初步发现令人鼓舞,并强调了从更好地了解不同器官部位的基因组学和免疫学可以获得的额外数据。除了CMS4亚型原发性肿瘤产生腹膜转移的倾向外,亚型转换在肝转移中很常见,但在腹膜转移中很少发生[9]。这些数据表明,在肿瘤微环境中存在免疫系统逃避或排除的独特分子机制,这是在不同解剖位置发生转移的基础。与相应的原发性肿瘤相比,腹膜转移中观察到的调节性T细胞上调提示免疫逃避表型,而其他转移部位对CMS2和3亚型的偏好更强,这与免疫细胞排除有关[16]。在另一项研究中,与原发性肿瘤和结直肠癌肝转移切除术后存活率相关的肿瘤浸润淋巴细胞表达模式与腹膜转移瘤细胞减退后的结果无关[17]。进一步了解肿瘤细胞与每个器官部位的微环境之间的复杂相互作用可能会导致患者更好地个性化治疗。

小结
 
总之,随着新型免疫治疗试验的开发,更好地了解免疫微环境在腹膜转移中的基因组差异和相应影响可能会显著改善患者的预后。将这些信息与肿瘤侧向、原发性肿瘤分期和组织学发现等传统危险因素的知识相结合将有助于指导治疗决策,并将转化研究目标纳入这些试验将加速下一代疗法的开发。
