循环肿瘤 DNA(ctDNA)被认为是微小残留病(MRD)的替代标志物,也是确定结肠癌复发风险的预测标志物。
如果不进行全身治疗,根治性切除术后可检测到 ctDNA (MRD阳性)的结肠癌患者复发风险显著(95%-100%),这表明MRD阳性不仅是复发的高危标志物,也是衡量疾病持续性指标。

如何在常规实践中使用ctDNA进行个性化治疗?
在将ctDNA确立为强生物标志物之后,使用ctDNA指导结肠癌辅助治疗相关的前瞻性临床试验是可预知的下一步(表1)[11-12]。由Tie等人领导的DYNAMIC试验比较了肿瘤知情、ctDNA引导的辅助治疗与II期结肠癌术后的标准治疗[11]。ctDNA导向治疗的2年无复发生存率(RFS)的主要终点并不劣于标准治疗,这让人想起 ctDNA 引导的方法也许可以在不影响II期结肠癌RFS的情况下限制辅助化疗的使用[11]。PEGASUS 是一项多中心前瞻性试验,对高危II期或III期结肠癌患者使用ctDNA引导的辅助治疗[12]。MRD阳性患者接受CAPOX治疗3个月,MRD阴性患者接受卡培他滨治疗6个月。在135例患者中,35例患者术后可检测到ctDNA(26%)。尽管 CAPOX组仍具有持久性ctDNA的患者接受了FOLFIRI。中位随访21.2个月后,35例患者中有14例(40%)观察到CAPOX或 FOLFIRI后的血清转化。有趣的是,MRD阴性III期和高危II期患者尽管使用卡培他滨进行了降级治疗,但复发率较低,为7%。需要注意的是,与基于血浆的检测相关的假阳性检测结果可能导致过度治疗;7例患者在完成 CAPOX和FOLFIRI治疗方案后表现出持续 ctDNA 阳性,但没有疾病复发的证据[12]。

ctDNA能否在不影响RFS的情况下识别出可能无法从辅助治疗中获益的患者?
根据现有的前瞻性数据,可以考虑以肿瘤为依据的、ctDNA引导的治疗以帮助确定可能无法从辅助治疗中获益的II期结肠癌患者队列。然而,我们需要来自更大规模的3期试验的长期随访和前瞻性验证数据来巩固这些证据。在结肠癌辅助治疗的MRD领域,已经进行了多项以ctDNA指导管理为重点的前瞻性试验(表2)。不幸的是,COBRA研究(NCT04068103)已经终止,因为与肿瘤无关的检测的假阳性ctDNA结果高于预期,这可能归因于甲基化和表观基因组标记物可能限制了检测的灵敏度。理想的检测方法应提供大于90%的灵敏度和特异性,并具有最小的假阳性和阴性检测结果;鉴于局限性(I、II、III)结肠癌的ctDNA数量有限,这一点很困难。参考文献:
