一些食管癌和胃腺癌(EGA)患者可能会受益于寡转移中更积极的局部治疗。
最近,人们对探索寡转移癌症患者的治疗方法重新产生了兴趣。这种兴趣受到系统疗法(包括基于生物标志物的治疗和免疫疗法)的进步、外科技术的创新以及立体定向放射疗法和消融等新型局部方法的发展的推动[1,2]。此外,当前的检测方法和改进的成像方式可以更好地识别患有真正寡转移疾病的患者,而不是广泛的IV期恶性肿瘤[3]。然而,寡转移状态的分类尚未达成一致。欧洲放射治疗与肿瘤学会和美国放射肿瘤学会都发表了关于寡转移疾病定义的共识声明[4,5]。普遍认为肿瘤负荷的临床相关性取决于原发肿瘤部位和组织学、生物学行为以及转移负荷和转移部位。因此,寡转移癌症的治疗应在特定肿瘤类型的背景下进行研究。
转移瘤切除术的作用
EGA是一个主要的健康问题[6]。年轻人中EGA的发病率显著上升,他们经常在晚期才被诊断出来[7]。尽管治疗取得了进展,晚期EGA患者的总生存期(OS)仍不足2年[8,9]。多项报告表明,部分转移性疾病负荷有限的EGA患者可能会受益于更积极的治疗。转移瘤切除术在寡转移EGA中的作用已在多个已发表文献中描述。这些研究大多数都是小型且回顾性的[10-14]。2016年,一项对39项研究的荟萃分析表明,480例接受肝转移切除术治疗的胃癌患者的OS为21个月,这比标准全身治疗所能达到的效果要好[15]。值得注意的是,西方(OS 11.6个月,9项研究)和东方(OS 21.7个月,30项研究)研究之间的结果存在很大差异,强调了这些疾病的生物学异质性。
最近,前瞻性非随机2期AIO-FLOT-3研究以前瞻性方式探讨了切除问题[16]。该试验的其中一个队列招募了患有方案定义的寡转移性疾病的患者。这些患者接受围手术期FLOT化疗,并在可行的情况下对所有疾病进行手术切除。该队列中60例患者的中位OS为22.9个月,其中36例能够接受转移瘤切除术的患者中位OS为31.3个月。鉴于AIO-FLOT-3的良好结果,德国正在进行一项随机3期试验(RENAISSANCE,NCT02578368),评估这种情况下的切除术。SURGIGAST是一项类似的3期研究,正在法国进行,评估寡转移性胃癌患者的手术切除情况(NCT03042169)。这些前瞻性随机试验的结果将进一步阐明寡转移EGA的定义和手术减瘤的潜在益处。
无创局部治疗的作用
手术方法本质上与术后并发症风险以及术后全身治疗延迟相关。非侵入性局部治疗可能为EGA带来更多希望,并且可能适合更多患者。立体定向放射治疗和大分割消融放射治疗可实现良好的局部肿瘤控制且毒性低[1]。MRI引导放疗技术可以实现精确的靶向肿瘤,同时使其他器官免受放疗相关毒性的影响[2]。此外,放射治疗和免疫治疗药物之间的协同作用(现已成为标准EGA治疗的一部分)进一步激发了人们对这些方法的兴趣[17]。
目前,寡转移EGA无创治疗的所有可用数据本质上都是回顾性的[18-21]。2期SABR-COMET试验将99 例来自不同原发灶的寡转移肿瘤患者随机分配至立体定向消融放射治疗(SABR)或全身治疗,结果表明 SABR的OS显著改善,但这些结果需要进一步验证。需要针对具体疾病的背景[22]。我们希望正在进行的3期ECOG-ACRIN研究EA2183将提供该领域非常需要的数据(NCT04248452)。EA2183是一项探索放疗在寡转移EGA中作用的3期研究。在这项研究中,经过大约4个月的全身治疗后疾病没有进展的患者将接受2:1的随机分配,要么继续全身治疗,要么对所有疾病部位进行巩固放疗,然后继续全身治疗。本研究的主要终点是OS。鉴于快速变化的治疗模式,该研究目前正在更新,以便在一线系统治疗方案的持续时间和类型上提供更大的灵活性。
未来的试验有望为疾病管理提供帮助
寡转移EGA的定义和治疗尚未确定。针对寡转移癌症患者的多学科治疗对于制定个体化治疗计划至关重要,以最大限度地提高生存率并提高生活质量。结合基于生物标志物的治疗和免疫疗法的全身治疗的最新进展、手术技术的改进以及新的局部治疗方法的开发支持进一步探索寡转移EGA的治疗模式。我们希望正在进行的几项试验(RENAISSANCE、SURGIGAST和EA2813)能够提供有关寡转移EGA管理的急需数据。
