编者按:尽管免疫检查点抑制剂(ICI)已经彻底改变了癌症治疗,但ICI诱导的结肠炎等不良副作用会破坏有效的治疗方案,并导致不可克服的毒性。本文中,德克萨斯大学MD安德森癌症中心炎症性肠病临床主任和粪菌移植主任、机构免疫治疗毒性工作组主席Yinghong Wang,对ICI诱导的结肠炎及其有效管理进行了介绍。

免疫检查点抑制剂(ICIs)已经彻底改变了晚期恶性肿瘤的管理、结局和生存[1]。然而,ICI结肠炎是ICI治疗最常见的不良事件之一,其通常发生于在给予第2~3剂ICI后,并成为有效的癌症治疗的主要障碍[2]。及时对ICI结肠炎进行临床、生化、影像学和内镜评估,并进行有效治疗是确保有效的癌症治疗和患者良好结局的关键。
鉴于ICI治疗是一个相对新的领域,我们对于ICI治疗毒性对分子和细胞机制的影响尚不完整。幸运的是,过去5年中,大量出现的证据极大提高了我们的认知和临床实践(表1)。尤其是,我们已经了解到,中重度ICI结肠炎的管理对于确保及时的临床反应/缓解、降低并发症风险、成功地恢复癌症治疗是非常有必要的。根据肿瘤学会当前的指导方针,类固醇仍然是ICI结肠炎患者的主要治疗方法,其次是对于难治性病例可进行选择性免疫抑制治疗(SIT;英夫利昔单抗[infliximab]和维得利珠单抗[vedolizumab])。出现某些“高风险”特征的疾病前30天内的内窥镜评估与激素难治性疾病病程和处方SIT相关[3]。这一实践改变已被多个学会指南推荐,并已越来越多地在当前的癌症治疗中被采用。新的内镜评分系统对于提供更准确的结肠炎严重程度的评估以及不良事件通用术语标准至关重要[4]。

ICI结肠炎的药物治疗
在ICI结肠炎的药物治疗方面,越来越多的证据表明,与长期皮质类固醇相比,SIT具有更高的疗效和更好的安全性[5-8]。对于结肠炎复发和顽固性(如高风险内镜特征)风险增加而不是类固醇难治性的部分患者,在最初的类固醇治疗期间(2周内)给予SIT治疗,可以进一步改善其结肠炎病程[9]。此外,在小样本病例研究中,乌司奴单抗(ustekinumab)和优特克单抗(tofacitinib)已被报道有效,但这些强效免疫抑制剂的长期安全性,尤其是与癌症结局的相关性尚不清楚[10-13]。内镜下结肠炎的愈合与较低的复发率相关,除了临床反应或缓解外,还可以作为更好结果的治疗目标[3]。粪钙卫蛋白也是预测内镜下缓解和指导治疗期间的有效替代标志物[14]。随着微生物组在癌症发病机制和肠道炎症中具有作用的数据越来越多,粪菌移植已被越来越多地采用。来自40多例患者的初步数据显示,粪菌移植作为一种补救治疗和具有持续长期效果的一线治疗,具有良好的效果[15-16]。该方法已被接受为一种有效的、研究性的替代方案。几个学术中心正在开展积极的临床试验(NCT03819296、NCT04038619和NCT04883762),或许我们即将从中获得更令人兴奋的数据。
对于初次ICI治疗后发生结肠炎事件的患者,由于再接受ICI后结肠炎复发率较高,因此不常规考虑恢复ICI治疗。然而,与CTLA-4方案相比,重新挑战PD-1/PD-L1单药治疗可能会降低这种风险,并大大延迟其发病[17]。同时进行SIT治疗和长期ICI维持治疗的策略已显示可成功减少结肠炎的复发,该策略或可在当前的肿瘤学实践中广泛实施,以增加肿瘤缓解[18]。
此外,尽管ICI的胃肠道不良事件风险大大增加(41% vs. 一般为11%),但是,随着越来越多的先前患有炎症性肠病的患者需要更积极的癌症治疗,ICI在当前的真实世界实践中仍得到了广泛使用[19]。与没有这种易感性的患者相比,有易感性的患者可以达到与之相当的肿瘤缓解[20-22]。因此,及时全面评估和适当管理对减轻这一人群的不良后果至关重要。
